Certified Community Behavioral Health Clinic (CCBHC)

What is a CCBHC?

CCBHC is a care plan that provides a full range of mental health and substance use disorder services to individuals with complex needs.  Typically, a person with mental illness must go through several different agencies to receive all of these services, which can be over complicated.  In contrast, each CCBHC clinic is an all-in-one provider of an array of services to individuals in need of care.

 

What services are provided?

A Care Coordinator will ensure that these services are being properly utilized and integrated.  Services available include:

  • 24-hour mobile crisis teams

  • Screening, assessment and diagnosis

  • Treatment planning and outpatient mental health services

  • Primary care screening and monitoring

  • Psychiatric rehabilitation

  • Outpatient chemical dependency services

  • Peer, adult, adolescent, family counseling services

  • Connections to other providers/systems such as criminal justice, foster care and more

  • Armed forces and veteran care

Why CCBHC?

CCBHC takes a person-centered and family-centered approach that looks at the physical, behavioral health and social service needs of the individual, since they influence the well-being of a person as a whole.  It provides a comprehensive set of services that promote recovery, wellness, trauma-informed care and a focus on whole health to serve individuals in need of care.  CCBHC serves adults, adolescents and children, as well as placing a special emphasis on active and veteran military care.

What are your rights?

In addition to the rights you have based on the licensure/certification of the program you receive services from, you also have the right to:

  • A free choice of organizations providing behavioral health services

  • Person-centered and coordinated care

  • Refuse a specific CCBHC service

 

We urge you to communicate with other health care providers you are receiving services from that you are in a CCBHC and that we want to coordinate our services with theirs to give you the best service possible.
