  Mobile Crisis outreach - Providing community response to individuals, families and children who are experiencing a mental health crisis

 


What is Mobile Crisis Outreach
The Mobile Crisis Outreach team provides mental health crisis services around-the-clock, 365-days-a year for children, adults and families. The crisis team’s mental health professionals and practitioners show up, when called on through the Crisis Line and Referral Service, to directly help people at hospital emergency departments, homes, law enforcement facilities, and other settings throughout the community with crisis intervention, assessment, and stabilization services.
Crisis Response Services
Community-based crisis services provided to adults, families and children including:
Crisis Assessment
  A face-to-face mental health assessment conducted by a mental health professional for evaluation of any immediate need for emergency services.

Crisis Intervention
  A face-to-face short term intensive mental health service initiated during a mental health crisis or mental health emergency to help the crisis person cope with immediate stressors, identify and utilize available resources and strengths, and begin to stabilize the crisis.

Crisis Stabilization
  The provision of short-term individualized mental health services designed to restore the person to a stable level of mental health functioning.
Service Coordination
MCO helps connect the person to:

Rapid Access Psychiatry
  Arrangement for psychiatric appointment within 72 hours.

Next Day Outpatient Appointment
  An appointment with a mental health professional the next day in one of the Northern Pines Mental Health Center offices.

Adult Rehabilitive Mental Health Services (ARMHS)
  Mental health practitioners who provide community support and skills training in the community.

Assertive Community Treatment (ACT)
  Intensive community services that provide medication management and support in the community.

Community Transition Support Services
  Community support and assistance for adults diagnosed with a mental illness.

Children's Therapeutic Support Services
  Community and school support for children dealing with emotional problems.

County Social Services
  Mental health case management for adults and children.
