Adult Community Based Services are designed to identify and meet the needs of adults dealing with the effects of a mental health diagnosis. The programs are behavioral in nature and intended to address deficits in areas such as self-care, social functioning, symptom management, medication compliance, and home maintenance.
Our goal is to increase personal independence and improve functioning within the recipient’s home community. The desired outcome is to prevent disruptions in functioning, unnecessary hospitalization, and to improve each individual’s quality of life. This is accomplished by utilizing the education and life experiences of a variety of staff persons.
Community Support Program (CSP) Services are provided to individuals diagnosed with a mental illness. Individual contacts between recipients and staff bring providers and recipients together in the recipient’s home/community. Peer group meetings to discuss mutual concerns are also available. Frequency, duration of appointment, goals, and length of involvement in CSP services are determined by working as a team with the recipient, their case manager, therapist, psychiatrist, and other identified support persons. CSP is available through Northern Pines in Todd and Crow Wing Counties.
Adult Rehabilitative Mental Health Services (ARMHS) is similar to CSP and is available to adults with Medical Assistance (MA) whose functioning is impaired due to the effects of a mental health diagnosis. This service is available individually and in small, short-term groups and it is designed to address the losses incurred due to mental health symptoms. ARMHS is available to adults on MA in all the counties of Region 5+, which include Aitkin, Cass, Crow Wing, Morrison, Todd, and Wadena.
Housing Support Services (HSS) assists individuals whose housing is in jeopardy and/or who are in need of assistance with the moving process due to the symptoms of a mental health diagnosis. The extent to which staff is involved in the transition process is dependent upon the referral and identified need of recipient. This service may include assisting a recipient in completing applications for housing assistance, identifying appropriate housing, meeting prospective landlords, reviewing lease agreements, actual physical moving process, etcetera, and sustaining independent living. These contacts are generally short-term, though recipients may transition to other Community Based Services. HSS is available throughout Regions 5+ (Aitkin, Cass, Crow Wing, Morrison, Todd, and Wadena Counties) with authorization from the county of residence.
All services begin with a diagnostic assessment completed by a mental health professional and/or a functional assessment to determine appropriate treatment. Records are maintained which include releases of information, an individual treatment plan, a record of contacts, condition of services, identification and contacts with other providers, and consultation with a psychiatrist. |
